Honors College Option Contract Form
This form must be completed and turned in to the Honors College Office to contract any Upper Division Course for Honors credit.

Honors College Directed Study Application
This application form must be completed and turned in to the Honors College Office before students will be allowed to register for Honors College Directed Study credits.

Directed Study Permission Form
This form may be completed by students. Once completed, it should be delivered to the Honors College Office for approval signatures and registration for credit.

Application for Graduation with Honors College Distinction
This form must be completed by students and turned in to the Honors College Office during their final semesters to graduate with Honors College Distinction.
